When an item shows no spot from where it drops from, but it reads at the bottom Ashes of Malmouth, does that mean it only drops in the Ashes of Malmouth area?

If yes, do some areas in Ashes of Malmouth have better chances of higher tier drops?
I'm guessing the answer is yes.... in the Dangerous Areas?

no
unless the item lists a source it just means the expansion needs to be installed,
expansion tag itself is not a source so you can farm it anywhere you want and get equal chance
