that's why healers are generally only annoying if you don't spot them or they come in groups of 3 (which seems to happen fairly often in SR
i can't remember the name of the cthonic blood lady bug healers, but you should have no trouble solo nuking them down when they spawn from a totem
To be honest I didn't spot the healer. There were 3 big monsters left running after me and I was damaging all of them pretty much equally, and all 3 kept getting fully healed. Maybe there was a 4th not running after me and healing the others?
Anyway, went back, found another totem there and this time I had no problems, so I'm guessing there were no healers

Basically you need to look for the supporter one, but Arcane is probably one of the nastiest if you rely upon buffs and will see that a fair bit in Ultimate.
One or two you should be ok to handle and about timing all your burst damage with their healing, when it is 3 or 4 (not sure if this is just SR) then it can be head on the keyboard time depending upon build :)
Off the top of my head, all the healer monsters are human-sized or smaller and they tend to be relatively stationary since both their heal and attacks are ranged. I suspect you're right about there being a 4th (and maybe 5th and 6th) enemy around. Hard to notice them if you're fighting three huge, dangerous melee enemies.
Monsters that actually heal themselves are different matter; most of those will chase you down in melee but they also can't heal themselves as fast as a caster healer could. Unfortunately they tend to be really tanky and/or really dangerous. Fortunately they are also very rare - limited to the one spot that unique enemy spawns at, or they are Nemesis for which you need max negative reputation, or you're in SR/the Crucible and have to expect that sort of thing. The answer in this case is to run away and come back with more DPS - usually uploading your character's save to Grimtools and posting the link here can help with that.
Recently I changed my weapon to one that's slower but the boost to grenado seems to compensate
