Nope, the very few additions are mostly easter eggs of no consequence.
You do keep your character, but what you have done in your main campaign will still be there. AoM campaign doesn't touch it in any way, and you can start it as soon as you finish the vanilla campaign.
If by "tree" you mean "mastery", then no. Classes are made out of 2 masteries, and a Death Knight is Soldier+Necromancer. If you're still a pure Soldier, you will be able to select Necromancer once you hit level 10 like you do with other masteries.

About 2) and 3), what you have to realize is that there are 3 Difficulties to Grim Dawn, Normal, Elite, and Ultimate. They're not quite distinct, however. The way the game is structured, once you've completed Normal, you're expected to go in Elite, then after finishing Elite, go into Ultimate. In a sense, the "end" is the end of the campain in Ultimate. You can stop at the end of Normal, but most likely you won't get to see the full potential of your character.
4) No, that's not how it works. There is not quite a "2handed" tree. I think you're talking about the Shaman tree, but what you have to understand is that Class trees are more flexible than that. If you pick Soldier, you don't have to use a Shield. If you pick Shaman, you don't have to use a 2hand weapon. You pick 2 classes, you pick a fighting style (melee, ranged, caster, pet) and you try to make it work. For exemple, staples of a character that picks Soldier are much more likely to be "War Cry" and "Field Command" rather than "character with a shield".
You can make a two-handed character with any class combination. I'm not talking about a top-build that can clear Gladiator 150-170 in 5 minutes, but a character that can finish the Campain (and the expac story) in Ultimate and reasonably kill almost all the Nemesis (maybe not the Celestial Bosses, though, these ones are really though).
So no, you can't pick a third tree, all you have to work with are two class trees and the devotion tree, but believe me, it's plenty to custom your character to your liking.
