You should see a button above friends list in lobby menu called "Join game" and by clicking on your friend OR set all your desired parameters and click on giant refresh button to list all open game sessions, including from other people too.

One player Hosts an Online game by selecting "Host Multiplayer" from the menu list, that defaults with "Single Player" as the option from the Main Menu, with these likely settings;
  • Internet is selected and not LAN
  • Auto-Party enabled - which automatically parties everyone in the game world
  • Instanced Loot enabled - each player gets their own loot
  • A unique-ish game name so if they need to search it they can.
  • A password that you share with your friend - so that randoms aren't joining (unless you want that)
  • If all your settings are as you want, click Accept, make sure the difficulty you want is correct and then press the "Start" button.
  • Once you are in the game world, your server is live and your friend can search your game and/or join you from the Internet Lobby Browser (detailed below).

The other player selects "Join Multiplayer" from the menu list that defaults with "Single Player" as the option. Make sure "Internet" is selected. Then click "Start" which brings up the Internet Lobby Browser. IF they are your friend in Steam they should see your name on the left pane of this window and IF the game is being hosted to the Internet then it should be displayed as "The Host's Steam Username (Multiplayer)". Your friend clicks on your name and clicks the "Join Game" button near the top left of the screen, enters the agreed upon password and is now in the game. If there is no password, then your friend immediately enters your game world.

So now if you're not Steam friends then you need to Search the browser for your friends game name and click the large refresh button (arrows spinning in a circle) to bring up the list of servers. It should be displayed if you did it correctly. And then click the aforementioned "Join Game" button and enter the password.

Not bugged, just a little badly optimized. The game was in development for quite some time that the engine got a bit old in the meantime. If you check the requirements for the game it says it supports Windows XP.

Funnily enough the x32 version might be more stable than the x64 if you got genuine hardware to support Win XP. This comes from personal experience as I've played this game on a Win XP machine.
