For a first playthrough does faction choice matter?

Depends on what class you're playing. Kymon's offer elemental stuff (mostly fire/lightning) while the Order is more pets and cold. If you have the Necromancer mastery by the time you come to choose Kymon's will have nothing to do with you.

If you are referring to just the first normal difficulty playthrough then as far as I know no not really.

Although I can't speak from experience since every character I got beyond the 1st difficulty I would always chose the same factions.

It only matters in Ultimate where the choice will be final. Each difficulty, for kymon/vigil, and barrowholm, you get to choose to be friendly to one and enemy to the other. So pick one in normal, other in elite, and choose one in ultimate. You'll mostly use them for gear augments, and those are reputation locked, so pick the one with best augments for your build.

Since you're on your first playthrough, definitely pick the opposite of what you decide in elite just so you can unlock all the blueprints, and the gear illusions if you have AoM expansion.

Each side also opens up an exclusive boss during their quests that can be farmed after quest completion. Kymon opens up the map in one area

In my first playthrough should I bother with the faction quests?
Yes, because they still give you XP and higher rep with them will allow you to buy their faction gear.

Both sides do have a gun, but the gear caps out at 70, so it won't be used endgame unless you lack the expansion

You can still look at the shops they have if you lack the rep, just can't buy until you do.

If you are a "completionist" or like myself have some level of OCD, then the first run is when you should purposely do all the dumb choices. Then in future runs, make the choices that are right for your style, so you get rewards at the level you can actually use them.
