i think you should mixed the damage, so you wont have problem with chtonians, you can choose elemen vitality, chaos, cold or bleeding.
For Vitality PB devotions Bat is pretty nice to start and you'll want the extra resist reduction from Manticore. Wendigo is a classic tier 2 devotion for this kind of build and pretty good for attaching to Bloody Pox. You would eventually aim for Dying God and/or Will of Rattosh. Will of Rattosh might be better to start with since it doesn't have a big downside and it can significantly improve your life leech in difficult fights.
Sorry to say this but forget that nonsense and just pay attention to your resist reduction.
Edit: http://www.grimdawn.com/forums/showthread.php?p=679744#post679744 has some older Witch Hunter Phantasmal Blades builds listed. You could look at those for ideas - but you'll need to adapt things for the gear you have/find and some of those are getting a bit dated. Actually reading the threads (not just the OP) can help a lot in figuring out how the pieces of a good PB build go together even if the particular set-up isn't the best anymore.
