Vanilla game gets you up to lvl 85, start doing the campaign on next difficulty (elite) if you haven't.
besides, hunt for loot.
first: you can also do the challenge dungeons (where you need skeleton keys) like steps of torment, port valbury, etc.
second: farm for nemesis bosses (unique bosses)
and it's really recommendable to get the DLC's, especially on sale, to have access to crucible arena or in forgotten gods the shattered realm arena.

2) Have you found the Hidden Path or been to Port Valbury? What about the East Marsh?
If you have filled in the map and found the secrets, then it's time to move on to Elite and work on getting your reputations maxed to unlock new gear and get access to Nemesis monsters.

get to Ultimate (2 more playthroughs) it will have a little unique/secret content

lvl an alt character? the game has like 36class combinations available and each of them have multiple different routes/"leveling options", start on the 2ndary char madness
(ps is you like the base game seriously consider the expansions, they are great and add a ton extra)
What are those areas you mentioned not sure if I did them I mainly focused on the main quests. Which sidequests/quest lines would you recommened are the most rewarding and enjoyable. Also should I do them on the next diffiuclty or just get them done now? I'm lvl 42.
you asked for the areas which roland mentioned, means you haven't seen them already and so you miss out a lot of content too.
then as i said farm for some gear in the challenge dungeons, which are a good challenge and get a feel for the game (different locations/bosses different damage types, etc.).
and after you feel like not getting any more experience, proceed to the next difficulty.
Against the argument some people will tell you about leveling an alternative character, don't do it, even if you don't 100% like the char.
Reason: Stick to one char and play him through up to ultimate and lvl85/100. Because after you beat and unlocked everything with your "main char" you will be much faster leveling other chars and it's less frustrating since you know what to consider! This is mandatory imho.

Other than that I'd personally just go to the next difficulty and fly through it as fast as you can so you can enjoy ultimate. That's really where the game shines IMO.
Worry about getting every little thing done once you reach ultimate difficulty. You can always go back to elite to farm rep or resistances if ultimate is too difficult or if you missed something.
I'd recommend doing them on each difficulty (the Hidden Path gives both 1 attribute and 1 skill point per difficulty) so start on Normal/Veteran. First fill in the entire map and talk to everyone with an exclamation point over their head and do their side quests.
Second, here's the info on the Hidden Path if you want it all/need help, https://grimdawn.gamepedia.com/The_Hidden_Path otherwise start by looking for a path across the water, that doesn't show on the map, on the east side of the Lower Crossing (the first area out of Devil's Crossing).
The East Marsh is across the bridge east of Burrwitch. You'll need to go there for a quest for the Outcast and to finish the Hidden Path.
Port Valbury is accessed through a hidden area in the Conflagration to the North-East of Homestead, just look for a detonation point and then take the portal to get to the Port's Outskirts. You'll need a skeleton key to complete the whole thing and you should wait until you get the quest from the Outcast for the area to get the most out of it. If you killed the Outcast, then you can skip that area until a later difficulty in which you side with her (which gives better rewards anyway). Or go through anyway for loot and XP (there's both Lore notes and a treasure/kill room)
