So yes, the only reliable way to force a save is s/q. I do it on occasion, when I find an item I was looking for ages, the homestaed 'recover the cannons' quest etc.
Pretty much this. The game saves with nearly anything you do. There was a debate/poll(?) very early on in GD's development over whether to keep the Save button that was carried over from TQIT. Iirc Medierra said the game saves constantly, but Iron Lore or THQ decided to include a Save button so players would feel more secure that the game was saved before exiting. In the end the Save button was dropped from GD.
However you can lose some progress.
Happened to me a few times, the game got stuck on black screen randomly and i had to signout to kill it. Lost full crusible loot and a bunch more items that way. Very annoying.
Seems to happen alot in coop as well.
Atleast save wasn't corrupted.
Wild guess would be they just write to file and not flush it. Letting the OS flush when its comportable with it, thats why people struggle to know when exactly it saves.
