Also, if I move up to elite difficulty, will I still be able to access the Malmouth missions, or will I have to start the entire game from scratch?
The storyline resets with each difficulty increase. So if you go up from Normal to Elite or from Elite to Ultimate, you will have to go through the base campaign again. You will still have all your levels, gear, reputation, and devotion points (though the shrines will reset, so that you can get more devos).
Edit: also there are some places where you have to make lasting choices, like with the Outcast or joining either the Order or the Chosen, that you will get the chance to change your mind about. There are some ideas on how best to handle that if you want to look for them. I don't think most players have a good reason not to befriend the Outcast (and plenty of reason to be nice there), but anything I could tell you beyond that would be a spoiler.
