Ghoulish Hunger-what are the indications that it's working?
I'm not brand new to this game or anything. Haven't used Ghoulish Hunger before, decided to snag it during this latest run. Initially I assigned it to Menhir's Will, but didn't notice anything. After a bit of googling, someone on a forum mentioned the skill must be "active" for it to work...which if if they're correct and I'm understanding it right, mean it will almost NEVER work with Menhir's; since Menhir's won't pop until 33%, ghoulish won't activate at 45%-but as soon as I hit 33%, I'm shot up well above 45%. Unless I'm in a REAL sticky situation in which my HP drops down below 45% a second time. But that seems really silly. Is this intended/ how it's supposed to work?

But I'm not done yet. After reading that and assuming this is how it works, I put a point into Mogdrogen's Pact and turned it on. Went to test it on lower level mobs, with the ability active. Got my health low enough that even Menhir's popped off-33%, while Ghoulish requires 45%. Didn't see any visual indications, nor any buffs pop up at the bottom of the screen. Hard to test the attack speed portion. Is this skill even working?

You should see a buff activate on your buff bar. Most devotion skills use a generic blue icon, so you'll have to mouse over them and read each that appears.

After you took Mog's Pact, did you reassign the devotion skill to that aura?

Also, you correctly determined that defensive devotions are best assigned to auras which are active 100% of the time, so it's wise to put at least a single point in your mastery's auras since most builds use at least one or two defensive devotion skills.

just tested, works perfectly,
like rylock said, it uses a very generic missable icon, (blue with white "star" in, like most devotions)
but it works, if attached to perma aura/buff, it will automatically activate at40% hp,
if attached to skill/proc buff, it will only activated when that skill is in effect, like Menhir, greatly reducing the effectiveness of it
and depending on your weaps and current attack speed might be why you don't notice much from ghoul, also the hp leech, depending on damage taken(and dealt) can also hard to notice sometimes, but it does proc/activate

Ok, just realized my mistake. I was thinking I was seeing a proc from another ability in the buffs portion, not realizing it was using the same one. It's working.
