My guess is line of sight, as well as distance, cone of vision, and how the game feels.
Line of sight, whether you're facing it or not and Resilience aced or not. If there's a wall blocking LoS to it, even a thin one like a door, you're unaffected for the most part.
I think it also has to do with bodies/objects being more or less on top of it when it goes off. Many times I've had a shield or body fall roughly on top of where the grenade appears & have seen the detonation effect but wasn't blinded or deafened.
Edit:
Also noticed Cops (both Jokered & hostile) & Crewmates (both human & AI) will block the effects if any part of their model is visually covering it or even just their model's 'center'.
