Unless he is a bot. If he's a bot then he wears the default shirt which is the blue one, again.

The mod allows you to pick the shirt
While without the mod, it's random.
That's it.
After short testing and research all I can say,
without mentioned mod he changes his shirts only randomized and this happens only ingame so in the lobby he wears always the same outfit.
P.S. No idea what the mod is called cause it never bothered me which outfit he is wearing.

Actually he changes his shirt when you switch from main menu to inventory and back. Don't remember if you had to switch something in your loadout to have it happen but it 100% was a way to change the shirt to see all the variations aviable when Sanres was released.
Besides the simpole recolors Sangres also has 3 special shirts with lower chances to show up
Pink: 1:50
Snake: 1:50
Taco: 1:200
