Randomly. Generally they'll show up maybe every 15-25 minutes, though they might sometimes be tough to hear over the sounds of gunfire.
Anyway, OP, you need to think about the situation logically. Dreams aren't really prophecies, but writers can occasionally use them as such. For the most part though, the dreams represent what the characters think.
Eg:
Signifies that Hoxton THINKS someone else is betraying him. That, or he's remembering someone telling him this.
Signifies that Jiro THINKS Bain is holding back on him.
They're both most likely right anyway from other sources though, so I don't think those two quotes are particularly vital.
