Is it linked to sanitation? I dont know anything anymore, not since patch 18. I have a lot of new stuff to get my head around.
Yeah, I do miss that from the older total wars. You could move across your regions and see the coloured bars, had that risk feel to it. We create garrison armies now, and move them to areas we think we'll need them.
The garrison for the settlements itself, is more grabbing people out of the local population who can fight, than a stationed garrison.
In Attila, when you add a governor his unit joins the garrison, would be good if we could add/remove units attached to a governor. Maybe no recruitment directly, but an army could merge some units into the governors forces. In thrones they added the food cost, so it could work in a future title.
The only problems I've had with the children dying is when I send them to be educated, or at least as Rome, so I would question whether this is true or not.
I believe any child is vulnerable to infant mortality in the Grand Campaign, regardless of what they are doing. I actually think it's a good little feature, as it does reflect one of the difficulties of the ancient, and even fairly recent, world.
As a general rule, I suggest getting your characters married as young as possible, to give them the most chance to have as many kids as possible (limit is 5 per couple.) The older the parents get, the lower the chance of having kids, and if the woman is 50 or over the chance is 0%.

i think when they reach the age of 7 or 8 its save to say that theyre out of the woods and before that there is nothing you can do against it
Kids used to die in large numbers.
It's part of the reason the expected lifespan was so low. It's not that healthy people didn't live into their 70's or beyond, it's that so many died young it skewed the average. If you made it to 20 you'd probably have a decent lifespan after that.
And that's why there's an option to not display dead characters.
