How do you melee people?
I've spent an hour trying to sneak through the same ♥♥♥♥♥♥♥♥ area and failing everytime (I'm seen and shot). I don't understand how combat works. I picked up a door jack thing but that doesn't appear in my inventory, it only comes out to smash padlocks on doors. Can't I hit people with this?

Attack button without pressing aim button. (you don't see it in your "inventory")

Yes, the game does a fine line in not telling you what buttons to press. Very annoying. Took me ages to figure out how the crafting system works.
Pull back on the left analogue stick and hold down Left Trigger. I'm not sure if it works all the time or is context sensitive to when an enemy thinks you may be in that locker/cupboard. To be honest I prefer not being stuck in lockers as it's very limiting.

Does not seem to work with a 360 controller. I can lean back, bring the scanner up or down but can't hold breath.

Is the Alien in front of the locker? As I said it appears to be context sensitive.

word. under desks and shelving is the best.

Damn straight, I even prefer circling round containers or tables I can't hide under to the lockers.

Yes sometimes just inches away when it walks back and forth then pauses a sec before walking away.

Do you get an "LT" prompt on the screen? And are you pulling back on the Left Analogue Stick? Sounds like it hasn't noticed you, good times. If it knows you're in the locker you get no prompt, can't hold your breath and die. If it's not sure you're in the locker and it comes right up to the vents pull back the LAS and hold LT but you have to be quick, if you've done it right then the screen will flash slightly red after a bit. I don't recommend using the Motion Tracker when in a Locker and the Alien is nearby.

Never got the message to hold breath and I played the whole game twice

That's good to know but alas I can't help you any further, sorry.
Could be it just doesn't think you're in that locker so the context sensitive notification doesn't come up and the context sensitive action doesn't happen or it knows you're in that locker all along and is just messing with you.
