I don't get your point in asking this question, can you explain better?
You cant host ur own game. You enter a server that already has players. On a single server there are sub-servers (channels, mirrors) to avoid keeping thousands of players in one single area. You can join a group with friend and play together if you both join a same channel (you need change channel for it).
Apart from the dungeons and some special events, this is a world without loading screens, it is not divided into zones like, for example, Borderlands games, or TFD.
Story missions do not automatically create a party, but there are temporary events that group-up different players if they all joined to event area.
The world is big, after some time after game start there will be fewer and fewer random players, and in the end it will be a solo game. Not include some raid activities, its 99% solo game - enemies are easy (too easy and devs ask players with suggestions on how to make them more difficult so we can see some changes at release).
Technically you can only play with your group, it's just that in addition to your group, there can also be several other groups in the same mission area.
The reason why I'm asking if it's possible to play Coop with a friend without interference of other players is because I don't want to play PvP, I don't want to deal with cheaters and hackers, I just want to play with my friend without random players bother us... For pve servers: you can enable pvp mode if you want. Not sure but real pvp enabled only if both players enable pvp mode, and some events triggering pvp mode. Otherwise nothing happens when firing other players. Not sure if there are any penalties for dying on a pve server, so cheaters shouldn't be a problem.
