Question about difficulty levels and eye?
I presume it is normal=easiest and Deadly +6 is the most difficult?
So it goes normal>hard>dangerous>deadly>deadly 1-6?
And what about the eye, does it appear every time after combat or?

You got it. The eye can appear during combat, but not always. There are some other eyes too other than the nuke one (buffs appear on monsters). The eye doesn't do anything when its up other than give you a chance to get Sauron Nuke'd.

Does this debuff only occur when outside? I am curious what effect this system will have on instances and end game raiding.

No clue.......
Not a big fan of the eye and everything that comes along with it. Heck SSG couldn't even list the difficulties in their proper order. Two steps forward, one back.

The debuff only occurs in solo instances or on the landscape. It will not effect instances or raids whatsoever.
