4) It's possible for multiple team members to earn a medal on a mission. I know in some missions it's possible to earn medals for 3 of 4 team members (particularly mission 7). I don't remember getting medals for all team members in one mission before but it could be possible.
Some exceptions to overall kill count are:
a) Stealth kills (SKs) - I don't know what they count for but I can tell you what I've read/heard. I've heard that knife SKs count for 5 normal infantry kills. I've heard that knife SKs are more valuable than gun SKs (possible with MP5SD or M16 Colt Silenced Sub-machine Gun). I have no data to back up any of this. I CAN tell you that I have had instances where getting ≥6-7 stealth kills (potentially 30-35 kills by that logic) has gotten me nothing.
b) On mission 1, I have been able to get Foley the first medal (AFSM) just by giving him all the kills from the bridge onward which I presume is <30.
c) Some missions appear to be harder to earn medals on (particularly missions 2, 4, and 5). I spent roughly an hour to exhaust all the kills from the spawn point at the starting gate of mission 2 (it spawns 1 enemy every ~30s) and I ended up with ~70 kills each for Bradley and Foley plus a few vehicle kills each and I didn't get any medals.
2) Vehicle kills - Getting ≥4 appears to give a good shot at earning a medal. I know you need genuine mobile combatant kills (BMPs, SCUD-Bs, MI-24 Hinds, T62s, T72s, ZSU-23-4s). Next, although they add to the total vehicle kills, I'm pretty sure civilian vehicles and Ural troop transports do nothing for earning medals. I'm not sure if SAM sites and the MiGs on mission 3 count but I think so and I doubt it hurts your chances.
1) Overall mission time does not appear to affect medal earning potential.
2) Accuracy seems to matter but I can't give you accurate cutoffs. I don't know if headshots matter.
3) Going MIA ≤2 times (getting killed/downed) does not appear to affect medal earning potential. Times hit does not appear to affect it either. Beyond that idk.
4) Using weapons to earn skill stars seems to help. For instance, Bradley's Combat Engineer skill will not increase without using C4. So once Jones has 4/4 stars for Combat Engineer I give the C4 to Bradley on each mission. Earning Combat Engineer stars seems to help Bradley get medals. Some skills come naturally, some skills need work (Foley and Jones rifleman) and others (pistol) don't. I've noticed that you do not need to use the pistol to skill stars. I've seen Foley's pistol skill increase to the max by mission 13 without even using it.
5) On mission 15 (assuming you have a Legion of Merit) killing Aziz is an automatic MoH. For my 3 MoH the breakdown was as follows:
Bradley - 31 inf, 7 vehicles, 11 times hit, 1 MIA
Foley - 44 inf, 6 vehicles, 15 times hit, 1 MIA
Connors - 4 inf (Aziz, his two guards, and 1 random inf), 0 vehicles, 0 times hit, 0 MIA
6) I have no idea how rank (e.g. - Major, Colonel, etc.) or experience (e.g. - Guard, Hero, etc.) is earned or how it plays into anything.
7) Finally, as you may have picked up, I generally went with a "Go loud!" approach. From what i've seen a stealth approach will not earn you more medals.
