Would be interesting to find out what the exact conditions for her appearance are.
About Quelana's dialogue piece. Probably she says that because she hangs around in dangerous places where mostly monsters live. Or might be because of a spell/magic/presense/your idea, theres no definite answer.
And like Spooky said, time is distorted. You can find Havel's armor in the secret room while Havel is still alive.
she usualy appears when your pyro glove is over +10.
Which leads me to this revelation. Pyromancy flame and/or possession of Quelaags soul and/or owning a Chaos pyromancy (thus, having the knowledge of Chaos, Sisters or pyromancy) is probably the catalyst for making Quelana 'visible' to humans (along with being the trigger for moving Quelana there, but I mean lore-wise and not gameplay-wise).
Salamar (or whoever the Pyro-dude's teacher was) probably met her this way too (purely my speculation)
MMm, the thing is, the clothes say ""Worn by the witch Quelana of Izalith", so.... thats what make me wonder about her appearance.
Her brother, that boss, is keeping the only thing that remains of her, her clothes, to remember her by. It's why he's enraged when you disturb them.
