Not relighting the Firelink bonfire is a bit of an annoyance, but it's not really worth restarting the game over, since you can just warp from somewhere else there if you need to go to New Londo.
In my opinion, keep playing without referencing the results, I think not knowing what things will lead to makes games interesting.
After defeating Ornstein and Smough you need to drop down the hole behind Firelink and place the Lordvessel there. After that the orange fog will lift.
Well the thing about decisions... I randomly answered yes to Laurentius and now hes gone so I don't have any way to upgrade my pyromancy flame.
Don't worry too much.
Which one is that? Quelana showed up in game before I even really knew what to do with her. I've since been back to try and find her but shes gone.
That's kind of part of the game though. It takes until your second playthough to figure out the right way to deal with NPCs.
