I had to disable DEP to get the game with dsfix to work.
do you even have a dinput device plugged in?
all these other so called fixes are not even related to dinput, so ofc they will not work. dinput is a component of directx. if a directx function has become broken or fails then the logical place to start is with directx.
you have to reinstall directx or use the redist appropriate for your OS located in your game's subfolder.
I believe you need DirectX 9 for Dark Souls. If this is where you originally downloaded it from and it fails to install, you probably already have it installed.

You would obviously just drag and drop them into the Data folder and overwrite the DSfix 2.4 files in there. Do not change any settings in dsfix.ini before launching the game please. (these only work with dsptde 2.0 steam beta, so make sure that is still enabled)
*EDIT* DSfix 2.2 was for GFWL, I removed it from the list but if you downloaded it, just know that it won't work with the steam beta.
