Besides the usual game mechanics.
Keep in mind the game is capped at 30FPS, with some drops in unoptimized places, that increases the latency and input lag.
That's one of the few things I feel DS2 actually improved upon. It messed up a lot of different things, but its general UI design is better than DS1.
I have a decent monitor, 23 inch or something and a 5ms response time.
This game doesnt have noticeable input lag. The problem is on your end.

Well, my monitor is rather old. Been using it for atleast 8 years by now. So it's just connected by normal old-fashioned cable. Actually a cable and an adapter cause its cable jack doesn't match with my graphic card.
Although i'll admit when I bought it I was specifically looking for something with as low response times as possible.

I suggest you re-read what's stated, it clearly says it doesn't affect everyone, and it's more common on consoles.
