Dark Souls won't run
Prior to this, my game has been running fine until it decided to up and ♥♥♥♥ itself one day ;~;
When incident occurred, I was in windowed mode and tabbed out, and now after it crashed it refuses to run; verifying files always ends the same - completes with 1 file reacquiring but never reacquires it.

Tried fresh install, tried forcing steam to update its files, tried altering settings...

It will run if I remove dsfix and friends, but I refuse to play it in that condition.

Also, now whenever I try to boot it up it reads "The application was unable to start correctly (0xc0000142), click OK to close the application." and tries a "First time installation" as it boots up, clicking OK on the warning does not close the game and it crashes almost instantly.

ダークソウルはもう死んでいる ;~;

The 1 file missing thing is normal
But,the application unable to start is definitely not normal
Have you tried renaming the moVvw file?

Something tells me that installing game to another partition may help.
Otherwise I would try to roll back OS and scan for malware.

Cant tell for sure, sorry.

Just tried that now, and it just crashed faster.

Okay so, after rolling back my OS, the only issue I'm having is it crashes whenever I try to use dsfix with it.

Disable in-game Anti-Alias - it interferes with DSFix
If that doesnt help - add executable to Windows DEP exclude list and see if it helps.
make sure it's dsfix, that you're not using the dsptde2.0 beta, and that you've extracted all the dsfix stuff into data, sometimes people forget the dll or the dsfix folder or stuff

What Windows do you run? 7,8,10, XP, 95? xD
Windows 7, I've been playing it fine running dsfix and everything but then it suddenly refused to work after one particular crash, which is bloody annoying, I might even have to install a different OS if this keeps up...

Usually if I tab out and do other stuff it's fine, but occasionally it can crash, and this time it broke it. ;~;
Update: Remembering Sajtt's mention of "make sure all files are properly placed," I reapplied them from the zip folder into my DATA folder and merged files as not to lose my texture mods or settings, and it's working now. ^.^

Not sure how a crash can somehow modify a file not directly linked with the game, but hey-ho, thanks for the help guys.
