I use tamamy drivers with x360ce and xpadder. Was a bit difficult to get it to work but now it works just great.
Vibration.
When using DS3 controllers on PC, turn off vibration if possible. I've had crashes and even BSOD's (WinXP lololol) because of that.
SCP and even MotioninJoy gave me problems with vibration enabled. Turn it off and report back.
Either inside the program you're using or inside the game itself. Every game comes with that in the options. You have to load your game to go into the options in Dark Souls. Don't know why you can't do it from the main menu...
No problem. This is the kind of problems that can emerge when you try to do exotic things with hardware that shouldn't be used on PC.
The people who code the emulators and programs for us to use the DS3 most likely base themselves on controller models they own themselves.
As most people know, companies like Sony like to update and upgrade their hardware and software sometimes. This includes controller as well. Sometimes, updates are applied and shipped. These "new" controller works perfectly as it should for the PS3 but can be interpreted differently by our emulators, which in turn leads to weird behavior, like vibration killing your controller driver entirely.

It also says Host Address: Disconnected, but I figure that's because I'm on USB instead of bluetooth.
Bottom of your controller is one of those "hard reset" buttons. You know, the ones you gotta' press with a paperclip or a pin or something? Poke it for a few seconds. Resets something in your controller. I did it, and when I connected it, I got a proper address in the monitor, and the vibration worked with SCP DS3. I figured it out because I had originally used my other DS3 controller and IT had an address in monitor, but this other one didn't. So I figured it must be the controller itself. Reset it, and it worked. Now I can use vibration in Fallout new Vegas with SCP DS3, so it should work for Dark Souls too.
I just bought a new Dualshock 3 and downloaded SCP. The controller only works in a couple games (GTA V, Fallout 4 - but not Dark Souls 3) but I don't have vibration either. The SCP monitor doesn't show an address, it just says Host Address: Disconnected (but it shows a connected controller in port 1). I tried resetting it but to no avail.
Recently, Windows has been trying to automatically download another driver for the controller, which when active fights with SCP. I tried deleting it, but it comes back every reboot. SO, if you locate the second driver in Device Manager and DISABLE it, that fixed the new problem for me. I mention because this recent issue had MGSV working right, Fallout 4 with no vibration, and Skyrim/ Fallout: New Vegas / L4D2 not recognizing the controller *at all*. I made sure there were the 2 controller drivers, with the second being installed but disabled, and that fixed it for me :)
I finally had the controller to work installing SCP server, but i cannot set the controls right. When i move around with the left analog stick or when i click it or when i click it while i move sometimes it drinks the estus, somethimes changes my weapon or shield. Its not possible to play, it seems like something is set wrong.
