I guess that's technically an example, but it's not like there's any downside to keeping items.
You can still choose to just go other routes sometimes.
I was more curious if there was anything that you can permanently miss, or mess up.
No. Even if you mess something up, next NG everything is reset and you can do it again/a new.
In particular, I missed out fighting Kirk in the Depths this playthrough,
so can't get his armor until NG+.
I figured that wouldn't be a problem,
but it did make me to think to ask if there's any 'forever permanent' stuff.
and also a 'checklist' of stuff to do before NG+.

but what if you quit a covenant, try to lose 'reputation', or things like that?
Couldn't you come back later and regain items after some kind of 'reset'?
Or is there really no way around it?
And do covenant awarded spells have the same 'one time only' limit?
For example, a chaos servant can't get bonus chaos pyromancy spells,
for attunement stacking each ng+?
Wow, that's got to easily be the worst trade with Snuggly the Crow:
two healing consumables that are slightly rare and will be back next playthrough,
for an item you'll never get again on that character.
I don't think any other item requests on her trade list[darksouls.wikidot.com]
are unique items awarded from covenant rank.
Covenant awarded spells are the same. This is why you know that someone that casts 5 Great Chaos Fireballs or uses Darkmoon Blade twice is cheating.
Quitting a covenant and regaining the rank that granted an item does not grant that item again. You get the item once, ever.

I'll be sure to be careful not to lose any as I go through NG+s.
