Dusk of Oolacile. Dead for no reason?

So Just finished beatting Manus on NG+ (Damn 10 minute fight. TY Faith healing spells!) And Dusk is nowhere to be found. I searched for a while looking for her but she wasn't there. I warped to the Sanctuary bonfire and even the Mushroom gal is gone as well with 3 Elizabeth's Mushrooms where she used to be. Did I skip something? I thought I did it right, I mean, there's not much you can do to veer off the path in this DLC area. It's a straight line from start to end, with only a few items in side rooms/areas to find.

The Question: Is this a known bug of some sort? I did buy all her spells and such before starting the area so I didn't expect much else after completing it, just did it to do it again on NG+. Was there something I missed or did wrong to have this happen?

Any input is appreciated to avoid this in my next run.

Im not sure why Elizabeth was found dead, but Dusk doesnt return to the Oolacile Sanctuary. If you want to see her again you must go back to our time in the Darkroot Basin and summon her.

Edit:
Just looked into it for you and Elizabeth will be dead if Dusk is dead. Perhaps you hit her while she was on the floor in Manus' boss room, or her summon got hit by a crystal golem in Darkroot Basin, or you just murdered her and forgot.

Still check Darkroot Basin for her summon sign in case this is just a glitch, perhaps from a NG+ problem or any other. I've heard of weirding happenings.

Thank you for the information. Somehow she died as her summon is no longer available. This is unfortunate. I will pay attention more my next run. Thank you for your help.
