Mod idea: turn-based combat
So, in short, it would be a mod that completely disables stamina regeneration for both player and enemies and even bosses, only recharging it when it's their turn. Player could roll, attack, walk, and do everything else he could normally do, but all that (including walking) would cost stamina.

No. you would be better off getting RPG maker and just creating 16bit sprites of the all NPCs

It definitely would be a whole new game. Every enemy encounter would be different and, I dare to say, challenging, especially at higher NGs, thus requiring planning and careful preparation. Of course, there should be an option to flee, returning to ordinary Dark Souls movement and mechanics. Trying to attack would put game into combat mode again. That's my vision.

Rolls could be implemented as mechanics granting invincibility for, say, next one enemy attack. Rolls wouldn't be very useful and even needed though because most ordinary enemies can be defeated with one stamina bar, but that would leave player open to other enemys' attacks.
