Nightmare mode impossible.
So I tried nightmare mode and yes, a few times I couldn't finish even the first level due to poor controls that often kill me (this was already during the first time I played it but then it saved).

So I wonder, does nightmare mode NEVER save? Or does it save in between each big chapter? Because if it doesn't and you have to do everything in one go, I believe the developers should assume the incompetent controls of the game and make it save in between each chapter, because there is no way I'm going through all of it when 95% of the mistakes during gameplay are due to the game and not because of me.

Also, come to think of it, it was generally a nightmare mode (because of the errors) all the way, but I did like the story a lot and obviously I already know the alternative ending. I just want to play through it again, but not if it's all in a single go.

There's a checkpoint at the beginning of act 2, when the Rat wakes you up, and another checkpoint at the beginning of act 3, when you enter the military camp.
If you leave the game after having going through a checkpoint you have to re-start nightmare mode from the beginning. So when I passed act 1 I left the game running to get some rest.

I finished this mode after many tries. I played with an xbox 360 controller because I think the controlls are more optimized for a controller than for the keyboard. Perhaps it might help you.
I guess one of the biggest problems was the jumps. Sometimes when I run to pass a big hole the hero didn't want to jump and fell to death. One hint is watching a playthrough on youtube when you play so you can anticipate where are the zombies, if the next area is safe or if you have to run...
BUT THE MAIN THING IS : YOU MUST FIND EVERY HEALTH AND STAMINA UPGRADER BEFORE TRYING. 6 lives can save you a stupid death when you miss a controll and you are attcked by a group of zombies. With the revolver and the axe you can survive but with only 3 lives, it is very tricky

Concerning the walkthrough, act 1 is not very difficult. There isn't really any traps or situations that require much reflex. Act 2 is the much annoying part. It is the longest and the most difficult one. You will need to know by memory every traps in the Rat's maze. If you succeed you will die during the part with the helicopter shooting at you. Whenever you get through this act, act 3 is nothing complicated. 2 or 3 tries and it's done.

Well, personally I feel the nightmare mode is absolutely not worth your efforts and your time. I also played this game with an xbox controller, which is a lot more comfortable, however nightmare mode is none interesting. It's not any different from a normal mode except for the amount of the checkpoints. So, don't waste your time unless you're obsessed about the achievements. I'd give it a try if the gameplay was a bit different.

Well, tbh I wish I had xbox controller, since PS controller was unable to play on this game. And thanks for saving my time, sir. I think it's only have the different ending?

Exactly, the only difference is the ending, which is awful in my opinion. Glad to help you
