But I see your problem. As you stated: takes 10 something hits to kill an enemy. That means you are not using heavy attacks right ? I mean the light attacks are pretty much useless against anything start using heavy attacks and you will shine.
You see, that's the problem - I am using the heavy attacks. I wind up, hit the enemy, block a hit, and it just goes like this in a circle for a good half a minute or so. And guess what, there's 4 other undead in the room who keep spitting acid at you when you're still fighting the first one. It's frustrating as hell to the point where I just start spamming fireballs at them and using up most of the offensive scrolls.
The lich can heal himself, so it's important to hit him as hard as possible, as fast as possible. The best method for that depends on the build you're running, but if you're having issues with that, then you may have spread yourself too thin. The game becomes significantly easier if you invest in any particular skill tree early on, and stick to it.
If you've got Flame Arrow, using it would be the best option. Quaff three or four potions if you have to, if your mana is still at 20. Build your adrenaline up, then incinerate him with the one-shot-kill adrenaline version of the spell.
If you're going sword and board, ignore the zombies or kick them off the sides, if possible. Hit him with power attacks until your adrenaline is built up, then kill him outright. All full-adrenaline attacks in this game are lethal if they connect, including arrows.

given that xana has merged with you to prevent you from dying that is indeed a bit of an unsuited thing to do to her. Funnily you can get through the game without killing the othe,r i just froze her for a moment opened the gate and slipped through :P
however I never used the form Xana grant kinda felt just way more complicated to play and be effective than needed.
