CHOICES REALLY MATTTER?
Just beat the game with over 70 hours, and was thinking about some of the horrible choices I made and I'll need to play the game again, my question is - what choices really matter, should I only focus on the main choices or even hidden quest will have a consequence in TW2?

I don't believe choices matter in the first Witcher. It did not effect my game play at all for TW2, at least to my knowledge it did not. There were no negative consequences that I noticed when I imported my save into TW2. I know the choice/ path you make it TW2 does effect game play/quests in TW3 if you choose to import a save.

In Witcher 1, I chose a romansewith Triss, and in TW2, I woke up with Triss, so I think she has a decision to do so. Besides, I don't know if you chose the option to load TW1 save, like me.

Choices that affect future: (Warning: spoilers!)
1. Defend lab with Triss/Defeat Chimera with Vesemir. If you choose to fight Chimera, you will fight additional enemy after completing "Clear the Crypt" in Chapter 1, but Vesemir will give you Red Meteor Ore if you ask him about Triss.
2. Trade with Elfs/Kill them. If you spare the Elfs, they will kill Break (Drug dealer) in Chapter 2, overwise Golan Vivaldi got arrested. (You can bail him out and he will give quest books for free). Affinity won't be affected by this choice.
3. Protect Witch/Leave her. If you choose to protect Witch in the end, fight with Beast will be more easy (he will not regain HP) and you will encounter Abigail later in the game. Overwise, she will die and you won't meet her later.
4. Learn the Truth/Be arrogant.
If you learn the truth about Raymond, you will frighten Azar Yaved and he will escape after fight. Overwise, he will steal the book that you have acquired before and this will affect chapter 3 ending.
5. Romance with Triss/Shani. No real consequences, but one that you don't choose will nag you in later games.
6. Spare the Werewolf/Kill the Werewolf. If you kill, you will miss one quest and this will affect ending of Chapter 3. Overwise, you lose a Mutagen's ability.
7. Free Selina/ Leave her. No real consequences, but you can do the nice thing. When you do the Outcast quest, he will let you pick the reward, pick the cheap thing. Later you can free Selina with this item.

Yes the choices DO matter, but only to you.

The Witcher is about the choices you make. It is not about making the "right" choices, because quite frankly they're aren't. For every "clear" right choice there's always a consequence of that action somewhere down the line.

There's no "correct" way to play the game. The entire point of the game series is about choice and consequence.

Geralt always says he's indifferent to everything. The old "I'd rather not choose at all." But he can't help but get in the way and has to make choices every single time.
