How do I beat Chapter IV?

I've been wandering across the fields and loading screens for what feels like ages and I still don't understand what I'm supposed to be doing even after reading a guide... The Ripples quest tells me to talk to Lady of the Lake who says nothing. Alvin's quest tells me to talk to Berengar who says nothing. Berengar's quest just says he should be punished. Everything else is done or looks like a non-essential sidequest.

The guide said Berengar's quest can be bugged but can be fixed by starting a 'village battle' sequence, without telling what the hell that is, there certainly isn't anything happening in the village when I go there...

Consult Geralt's journal, Corlain:

Ingame: Press "J" -> Quests; Filters: Main Quests, Chapter IV. What can you read there?

It shows the Alvin quest "Maybe I can talk to Berengar later" and the Ripples quest "I should see the Lady of the Lake". Neither of these characters says anything related to these quests when I talk to them.

Weird. If you don't (and didn't) use any cheats, mods, the savegame editor etc.* then upload your actual save and post the link; I will have a look at Geralt's journal. Perhaps I'll find the missing trigger.

No I'm not using any mods. Oh yeah and the quest marker points at Lady of the Lake. The only dialogue she has is related to the ending of the Alina quest and to the grail knights... but I've done everything related to that too.

Fine, then you should follow my proposal and upload your savegame. We could play the game "Ten Questions - Ten Answers" instead, but I'm not very fond of it. With our pace it could take 10 days, especially if we belong to different time zones. In this time you could play the complete chapter IV again, fufufufufu.

It's even easier than I thought before. The crucial point is the first one (!).

• Go to the Vodyanoi priest and report that you have killed Dagon (trigger!).He will reward Geralt.
  
• Return to Black Tern Island and Dagon's temple where you'll find the Lady. Give her my regards - and talk to her. She will reward you too. And from now on everything should be alright.

Thanks, it's weird it doesn't show talking to the priest as a quest at this point.

LOL then I can't use the log correctly, I'd have thought I had done the phases which come before the current one. I have to keep that in mind.
