Some long - across multiple chapters - quests rewards you with weapons. You get a very good armor and good weapons (both silver and regular) from missions that starts in chapter 1 (to ch 5 - trophy quests, that rewards you with the strongest silver or steel sword), chapter 3 ( "won't hurt a bit" - but needs an item from chapter 1) , chapter 4 ( "ripples" + "heat of the day" for silver sword, "Armor" for armor ) and chapter 5 ("mud and velvet").

Or you can buy one armor in chapter 2 / 3. You also can create weapons at the blacksmith.

I myself didn't need to buy that armor and didn't made any weapons at the blacksmith. If you do all (or the most) side quests, you will have access to a huge pool of experiences, which is sufficient to have a high level. With this and (sometimes potions) the hardest difficulty wasn't "hard". I only have done the Armor quest and retrieved the weapons from those long quests. At the end, I got Aerondight and two G'valchirs. (I could get the stronger silver sword, moon blade, but the story behind aerondight appeals me more than that, so I picked the steel one instead).

If you make a certain choice in the prologue you can get three red chunks and forge a very good steel sword in chapter 1. Failing that you can get a really good steel sword in chapter 2 from the swamp (found as loot, no questing involved).

I have always found the standard silver sword good enough and never felt the need to upgrade it, although I think you should be able to infuse your default silver sword with three runes by chapter 3 if you pick certain rewards and such (not verified).

As for armor, there are only three of them. The default one you get in the prologue, the one you can buy from armor merchants (from chapter 2 onwards) and a special armor (the best) that you get through questing across two chapters.

Once into chapter two I have all the gear I need:

Steel Sword (found in the Swamp)
Silver Sword (default)
Armor (purchased from a merchant)

In chapter four I get a silver sword replacement, in chapter five I get another silver sword replacement, a steel sword replacement and an armor replacement.

I have tried to keep it as spoiler free as I can.
