Amadeus is right. You're probably trying to use a sword that the monster isn't vulnerable to or using a combat style that isn't appropriate. Check the monster section of your journal to see if you have information on that monster which will describe its weaknesses. There's usually info as to whether you should be using the steel or silver sword and sometimes even an indication of whether you need to use strong or fast attacks. Also, using the group attack style against solo enemies often leads to failed attacks. If you don't have any information in your journal, then you need to experiment - there are only 6 sword/attack combinations, so it doesn't take too long to figure out which one hits the enemy and does damage. Learning about enemies and using the correct sword/attack combination is a big part of surviving battles in every Witcher game.
Also keep in mind that unlike lots of games, this one doesn't have "button spam" attacks. In fact, spamming the attack button is a bad thing that will get you killed - if you click at the wrong time, you will interrupt your previous attack (usually you'll see a "too fast"message show up). You need to click once and wait for the combat animation to complete. If you've unlocked combos, then when the icon gets an orange aura/flame around it, you click again for the next move in the sequence.
EDIT:
Another thing that can cause this to happen is when you try to attack an enemy as they are "spawning" into the world. Give them a second to become "active" before attacking.
As you guys can see at the video I linked, a single click doesn't do anything, serveral clicks - sometimes - manage the sword to swing weirdly.
Thank you for your very supportive comment, but I wouldn't come here and beg for help if I didn't try finding an answer by myself or by researching. Those guides are very helpful tbh, but as soon as I'm not even able to pass through the intro part, they're totally useless.
I ask you guys to please take a look at the video because it was the only one guy I found that seems to have the exactly same problem as I do, altought he eventually got it while I always had it.
When I try multiple clicks, he kinda of swings it but still no hit or damage, just regular swing without getting the sword down (he swings it while the sword is still over his head, the same as in the video)
If it doen't work (again), I'm gonna record my own gameplay as soon as I'm able so you guys can have a better look at what's happening.
But I'm still looking for any help you could give me.
Here, the multiple Barghests need to be attacked with group style, and the attacks dont find place because they attack too. This is when you run away, make some space between you and the enemy.
Not a bug.
Not to mention sword type matters too.
Is this on hard? I recommend playing it on easy and breezing through if this is your first time.
Dont let the gameplay hinder the story.
