Let me explain, in Arkham City there is a pregnancy test found in one of the rooms that says positive. Then in the Harley Quinn DLC you find a lot of pregnancy tests where they all say negative. It was assumed that the positive one was just a false positive.

I haven't seen harley quinn holding her stomach (is that in the DLC for AK? or during the game?)

I just read that Sefton Hill confirmed it was a false positive. Can't find the original source for that quote though.

Main game When Harley frees the infected joker, the old one points a gun at her stomach and at the end of the game she is holding her stomach

If anyone remembers that old batman beyond show, there is a scene where is shows you that the twin girls in it are the daughters of Harley.

I hope they make a batman game of that, I would like a futuristic Gotham with splicers and all that.

Its only a speculation. As I remember you actually can notice a lot of pregnancy tests, saying that she failed it. It was also believed to be the reason why she went so mad in that DLC for AC, not just becuase Joker died, but also because she wasnt pregnant actually and couldnt continue his legacy. Its a shame that people dont pay attention to various details much nova days.
