I do agree they should've explained this better in-game though.
In AC, Joker says "take my blood... I wish someone would.... this stuff is KILLING ME!!! Hahahahaha"
So it wasn't a cure for Titan infection. That was cured, which was bringing him back from the Titan creature he had turned into.
The infection Joker died from was like a mutation of the after-effects of the Titan formula.
Thus, it was literally his blood killing him pretty much.
The Batman wiki linked here by lucasj1210 doesn't even offer a proper reason why Bats is still infected, just saying that maybe it was because he only used like half of the cure, or that it was further mutated by the Titan formula or something.
Should have been explained properly I thought, either way.
As I said, I still don't mind even if it was just the only way they could find to bring back Joker, because Joker has been a highlight of this whole series, except for in arkham origins, which was a bit lacklustre compared to the rest (but still great compared to other superhero games)
