I've seen some videos online with people playing as the "Fear Knight" from the true ending.

Does anyone know of a way to unlock it?

INSTRUCTIONS:
● STEP 1
Download this file:
http://www.batman-arkham.com/upload/Batman-Arkham-Knight-Mesh-Swapping-Mod.rar

● STEP 2
Open "Batman-Arkham-Knight-Mesh-Swapping-Mod.rar" and extract the file "Console.txt" in the folder "Binaries" that you can find in the main game's directory ("Batman Arkham Knight\Binaries").

● STEP 3
Look for a model you want to use from the file "Player Meshes.ini", copy the lines for that model and paste them into "Console.txt" that is now located in your Binaries folder (replace its content, by default it contains the lines for Arkham Knight \ Red Hood).

● STEP 4
Go to "Batman Arkham Knight/BMgame/config" and open BMInput.ini with the Notepad. Add this line:
Bindings=(Name="PageUp",Command="exec Console.txt")

● STEP 5
For Nightmare Batman or Killer Croc, make sure you have copied the mesh code into "Console.txt". To use the skin, you first need to trigger the cutscene where the character appears (Scarecrow's hallucination at the end of the game for Nightmare Batman and Joker's hallucination for Killer Croc). As soon as the game shows the character, press pause and press the assigned hotkey (Page Up is the default key) to load the Mesh codes. Then simply select "Restart" and start spamming the assigned hotkey (Page Up is the default key) immediately once the screen goes black. Use another savegame without exiting the game to play in free roam with the new skin.

I don't know what you saw, but that's not an official skin. It was probably a mod.
