I've had this happen a few times; each time, the combo was 50x or higher. Frustrating.

I see some strange stuff happen with the blade attacks in this game sometimes. I've had ninjas come at me with their swords missing and a weird triangle where the counter icon should be (including on Hard, when they're should be nothing). I once tried punching a knife thug and Batman kept recoiling with the knife attack sound playing each time. Sometimes getting cut by the first one and doing the blade dodge input (not the takedown) doesn't work and he gets cut anyway.

I'm trying to get the Requiem for a Killer Achievement (it's a bit of a grind, but I'm getting closer). I've taken to jumping over ninjas instead of bothering to blade dodge.
