Spoiler Alert!

It depends on your responses during the disucssion about Revan with Atton in your first encounter on Peragus. If you selected the responses that made Revan a dark side female, you won't see Carth. This is because, if you play the first game with a female Revan and go dark side, Carth shows up at the end of the Star Forge and tries to "save" Revan. Revan then either kills Carth or orders Bastilla to do it. So, with that ending, Carth didn't survive the first game and, therefore, can't be in this one.

Bastilla can appear as LS or DS so she'll be in your playthrough either way.

Just mouse over to see where you can find her:

There's a holocron on Korriban from her and she'll talk to Carth on Telos after Darth Nihilus has been defeated. You can not interact with her yourself.

If you were a male dark side, Carth would have survived because he runs away when he finds out you've gone dark and doesn't try to save you on the Star Forge. He only tries to save a female Revan because he's in love with her. Female dark side is the only game variation in which he dies. So, if you chose the appropriate conversation choices with Atton, Carth would be in the game. That conversation is literally designed to allow you to input how you played the first game. If you choose the responses to make Revan a dark side female, another admiral appears in Carth's place using one of the generic male heads and a different voice.

If Revan was dark side then Carth is not in the game. It does not matter if Revan was male or female.

True.
If Revan was a dark side female, then Carth died trying to "save" her. (Interestingly, they say there was a cut cutscene where a DS Revan that finished the Carth romance option could choose to stay and die next to Carth)

But, if Revan was a dark side male, then Carth was still left stranded on the unknown world, his fate unknown.
So either way, he's not appearing.

Hm, I was thinking he was in the game if you went dark side male, but obviously I was mistaken.

There's been a mod to restore it back in for quite some time -- but it's also in K1R as well. It's a great ending honestly and it really should've stayed in the vanilla game as an option.

Here's a video of the ending if anyone's interested:
