I'd probably go with 1 saber, I'd be tempted to take a double blader but I know my ceiling won't be super high so I'd probably better stick with the one. I've honestly forgotten most of the forms, but I'd probably go with something fast-medium speed, think like cyan or yellow if you play Moviebattles II (I believe the forms in the game are supposed to be the ones from other works but I don't remember which is which, except that yellow might be the first one you learn in this).
I'd be taught by Mace Windu because he's one of the best fighters and is a total legend (he' Samuel L Jackson!) I know Kreia talks about ancient Sith Lords who could beat anyone in a fight but they'd probably just torture and/or kill me if I wasn't perfect, so I'd better not pick them.
I'd be a sentinel. I'm not choosing that, but I know that's what my skill set would be. I'm not in super good shape (guardian), and I'm not very good at diffusing situations (consulor), but I'm good with tech stuff like that (sentinel). I suspect that most other people on these forums will say the same, because the people who will be on here and responding to this post probably are mostly people like me who use computers a lot and aren't in great shape.
After learning a bunch of cool stuff from Windu I'd leave the order (making sure to do so at least a year before order 66 so I can get lost on Nar Shaddaa or something). Because I don't want to get killed, and because I disagree with a lot of the Jedi's philosophy (mine would probably be about the same as Bindo, with small bits of Kreia mixed in).
After these handful of days of training I'd think I'm some kind of epic Jedi Guardian but using a green lightsaber. Thinking I'd mastered Soresu I get into a fight with a small group of Sith troopers but get capped off by the first shot fired.
Having said that, the form you start with and Niman are both good simply because then don't have any significant weaknesses.
As for lightsabers I would carry a single and a short. That gives you choices as you can use both or either one on it's own. Again, choose which is appropriate to the situation. The famous samurai, Miyamoto Musashi, apparently only used a katana in duels with single opponents but did use both a katana and a wakizashi if fighting multiple opponents.
Decent defence, better offence and no stupid acrobatics. The form most akin to European longsword schools.
Thoughts on other forms -
Form I - Too basic, too focused on preservation of your opponent.
Form II - Too specialized in offence versus saber. (Probably would be my runner up.)
Form III - Too specialized in defence versus blaster.
Form IV - Spinning, tumbling and jumping are dumb in anything that involves a melee weapon.
Form VI - Doesn't know what it wants to be. Tries to be everything but is good at nothing.
Form VII - Weeb ♥♥♥♥. Too reckless.
Anakin Skywalker, before he fell as a training partner. Some risk of waking the dragon, but I'd imagine he'd dial it back if I let him moan about Padme now and then.
I agree with what you said except about niman. Niman practiced by itself is as you said, but it is not a form easily mastered. It like juyo requires something to build off of. If you look at the truly great Niman specialists they have other skills. Darth Krayt has traditional Jar Kai and terriscasi to fall back on. Darth Maul had ataru, terriscasi. And exar Kun had jar Kai and trakata. I think if a master of multiple forms used niman to tie it all together he would be unbeatable, but this is just my opinion. Usually the form just ends up being a dull and useless form :)
