Should I by the DLC?
As I already have the Omerta - City of Gangsters base game in my library I am considering buying the DLCs also.

As I go through the description of the DLC - Damsel in Distress:

"DLC it shows NOTE: This DLC is available after successfully completing the first two missions of the single player campaign."

Is this something you unlock during the game or you still need to buy it in order to get it in the game?

Please let me know.

Thank you,

The Damsel in Distress, The Arms Industry and The Con Artist are small side missions you can do during the main quest. They all give some unique weapons and character you can add to your roster (still nothing that beats Loony however). You are basically just adding a new quest in the main quest. They are not bad, but it is just one mission from my memory.

The Japanese Incentive however is something that gives you far more bang for your bucks then what the other DLC give you. With the Japanese Incentive you pretty much get a whole new story. You actually have to do gang warfare with other gangs. The map is much bigger and completely new. You get new weapons such as Katanas, Flamethrowers and machine guns. However nothing in this DLC will carry over to the main quest. It is a standalone story.

If you are looking for just some cheap side missions to add to the main quest the trio is what you are looking. If you want a completely new story with some added gameplay elements and real gang warfare (best part really) Japanese Incentive is what you want.
