I tried one or more times by reloading my save thinking it's over and blaming bethesda then I finally succeeded to climb up.
Good for me I was playing on nightmare so I saved just before. If you didn't save and have the autosave disabled like me, that's strange you can't restart because you should still have an autosave at the beginning of the level?
I'm curious but are you trying to climb with the chaingun in hands? Because I wonder if it was the cause of this issue as I had it equiped when I got stuck myself, that's a huge weapon and the hatch is not large above the ladder.
Go to the game directory, create a shortcut of the Doom3 application, in property, add +seta com_allowconsole 1 in the general tab to the launch line. Then once you are in the game use tilde ~ and type noclip, you can now go through the ladder, once you are safe you can type noclip again to desactivate. It should solve the issue completely by putting things in order.
If not use the command when needed hoping you are towards the end of the game just like when it happened to me. I think this happens when you fight and jump a lot then take a missile which make you move and displace the player location in the game which would explain why you cannot climb stairs and ladders anymore. The noclip command fixes that. I hope it will help!
