Noticed you said make sure keys are set to do something is this something we need to setup or is it configured already?
However, in the input settings tab, I noticed that there was an extra input device listed in the drop down that states the name of the device... Even though "XBOX 360 For Windows (Controller)" was selected, there was an entry for my space mouse in the list as well.
Disconnecting the extra input device fixed everything.
So if you are having the same issue, be sure to check the drop down to make sure your controller is the ONLY device listed.
Ill try this out later.
I've got a UPS that appears as a game controller. I deactivated the device and now I can use the gamepad.

Also, the youtube video "[How To] Enable Vibration For PC or PS2 Controller Tutorial" links to a driver update for many types of gamepads (choose direct download or you'll get malware) that got the vibration function working for mine.
