As I said, set wepons to 1, choose Thorn as talent.
Your HP is fixed at 1, so one hit = dead.
You need to survive until the Boss appears AND kill the Boss.
I can't recall about +def, but movement speed was handy.

Duellist has the death door ability where if you more than 1 hitpoint you survive fatal damage with 1 hitpoint and go invincible for 1 second but cant regen health for 3 seconds.
That an extra layer of protection if you having trouble. Though it unlikely to be needed with thorns talent.
Explosive Vial passive makes the potions that make enemies explode on death drop much more frequently and causes its duration to double (from 30 seconds to 60 seconds).
The Pike passive causes a cone of damage to shoot out behind enemies you kill.
Both those combined means when you kill one enemy with thorns, a chain reaction of explosions happens behind it, often clearing the screen. They are passive talents so do not count as a weapon. Add in sage leaf, trolls blood, and be a dualist, and you can go a decent distance into survival with this (I finally stopped moving and let things kill me when the second boss spawned, much farther than is needed for this achievement). And of course once you level up thorns enough that it gains projectiles, it all snowballs from there.
Not really sure how this worked out, halfway to infinity I stopped trying to survive but I had somehow achieved immortality?
Decided to ride it out to the end and somehow was able to kill everything up to(and even some after?!) the "infinity" part started. I did get the achievement though which I was a bit worried about, I did not dare pick the evolution in case that would have counted as another weapon or something.
