Is DDO an open world mmo?
I haven't tried DDO, but I am curious if it plays like an open world MMO, for example if I wanted to go from Neverwinter to Myth Drannor on horse back or something like that, is that possible?

No, its totally not an open world.
Technically its instance-based game.
There are public instances (citty squares and some others), usually same for all players on servers (e.g. marketplace). But if there too many players (>100 maybe) another instance will be generated.
Private instances are quests themselvs (dungeons) and wilderness areas.
Most wilderness areas are pretty big and sometimes pretty well designed, but normally players just use them to travel between quests.

If we talk about Forgotten Realms, wilderness areas are Drannor, King's Forest, High road, Wheloon prison, Storm Horns, small part of Underdark and Sschyndarlynn.

As J.C. mentioned, no. Not open world. All instanced. Public areas, wildernesses, and quests are all instanced, limited locations (though some are quite huge). There are waypoints of varying types, from quest entrances, to teleporters, to guides, to guild ships, to spells and even items that can take you from one instance to another. But you can't just ride across Xen'drik.
Thanks for the information!
