Regrettably, this is a known bug of unknown provenance. "Unknown" doesn't mean there's anything invalid in your entry, just some kind of communication and/or processing glitch. It seems to clear up on its own in a day or three for no known reason.
I recommend shutting your computer down entirely and restarting and retrying. Shouldn't require reinstalling or verifying DDO files (though there's little harm in the latter).
And there's no huge hurry to redeem the code; it's good until April 23.
That has to do with the Name you picked and it's a bug like Doug said. Generally your options are; Invalid (name), taken or the 'unknown' which is a bug.
Beyond making sure you've followed the naming rules (things like no underscore etc....) just keep trying and it'll eventually go away.
Open the in-game DDO Store (Ctrl-S or find on menu). Click "Redeem Code" at the top. Enter the text dungeoncrawl
I've seen a few variations of this. I regularly get offered MotU and Shadowfell Conspiracy in the in-game store. I bought them when they came out via the DDO Market. And I frankly assume the Store hasn't been updated with the info from the Market. I definitely have them, though. Ran quests in both of them just yesterday.
If you have any real question, of course, just enter one of the quests in question to confirm that you have access to it.
You can always check the 'My Account' page if you bought stuff via the online marketplace or open up the in game store and over by your name is a downward arrow, click on it and choose HISTORY and then scroll back to see if you can find it.
I think the system see's the 99pt item as different from the normal priced one and thats why it shows up. I got the same for Ravenloft.
Came back to activate the code, but when accesing store ingame it asks me if i want to use browser, when i "Ok" it, it open browser with 404 error and other option is just "cancel"

You may've been caught in a catch-22. If you can't open DDO from Steam, yet DDO knows you're a Steam user, it will try to open your Steam wallet when you access the in-game store and will fail because you didn't start it with the Steam overlay running.
If you aren't a Steam user, it should use the internal browser.
That's my best guess at an explanation.
In the game's options is one for 'use external browser' and it's on by default (I think) for steam users (so you can use the steam wallet).
Just open the game outside steam, go into the Options, find it and uncheck it now the store will open up in game normal and you can use the code.
