There are a few elements to it.

Every 100 Favor earns you 25 DDO Points. (There are also bonus DDO Points for the first toon on each server to hit certain milestones.)
Gaining overall favor or favor with specific patrons grants you specific rewards. Talk to the spokesperson for that Patron when you've unlocked a reward in order to collect it. Talk to Nyx Durandimion in the Harbor when you reach overall (total) favor milestones.

As for acquiring it, each quest has a specific amount of favor it grants on Elite or Reaper. It's always a multiple of 3 because if you run the quest on Normal, you get 1/3 of that amount and on Hard you get 2/3.
We measure it from the Elite/Reaper end because that amount is ALL there is for that quest. It's not like XP where rerunning the quest gets you XP again (just a bit less). Once you've run a particular difficulty, you have to run a higher difficulty to gain more favor for that quest. And once you've run it on Elite or Reaper, you've gotten all there is.. until you Reincarnate (some kinds) and reset your Favor again.

Example: A quest that gives 18 Favor on Elite, will give you 12 if you run it on Hard. If you later run it Elite/Reaper, you get the other 6. You can never get more than 18 Favor for that quest in one life.
So you can run it Normal/Hard/Elite and get 6/6/6 or run it 'one and done' on Elite or Reaper and get the full 18.

If you go to the DDO wiki and look at Favor there, it'll show you by the different reps what you get for X favor with them. For example the Coin Lords give you an inventory tab at 100 favor with them.

400 favor over all unlocks the Warforged race, 1700 and something the 32pt build etc....

So you can look at favor as both a way to earn free store points and unlocking various bonuses in the game.

As a General Rule for Grinding Favor It is in your best Interest to not be too concerned with it on your first Character on your First Life. Objectively, once you have 2 Past Lives on it, you can open everything on Elite yourself forever after. If you want to be even more efficient use 2 Accounts, each with an Elite Opener, can then Farm Favor by making a throw away character, who can get full Favor on Elite. Then you can delete them and remake them to run the Farm again. While your Reincarnated characters continue on to play all the content at mid to higher levels.

To allow access using high level Reincarnated characters, you just goto the entrance and set it to Elite without going in. As long as it is set that way a character who otherwise can not select Elite, in that moment can. And that's only if you care about XP. The real cheese is that if you do not care about the XP you can just make the run with the High Level character and get Favor with your farming character just fine anyway.
