Random spawns in this game always seemed more than just random to me too. From what I've seen it's pretty commonly felt among players. Adds to the DnD feel imo.
There has always seemed to be a hidden factor that the devs have control over.
Anyone who has had to ransack a named chest more than once can probably attest to this....but the ransack itself makes it hard to get enough data. I don't think anyone has figured out if there is a hidden factor or not. But absence of drops has resulted in bug reports...
I've heard people say they only ever found a certain item with their first character. I seemed to have that experience myself many years ago. For one item, player A has to try 50+ times (literally), while player B finds the item 2 or 3 times in a row in their first 3 runs.
It's also been my experience that you won't have that bad luck with everything.

I'm aware. That setting shouldn't affect a named mephit spawning. And as far as I know, it doesn't affect any named item drops, just random loot. Also, there are many named items that aren't class specific.
There's a different chance to drop for different items. Without searching through all the player accounts over the years, I'll say: I think it's possible that different experiences in named item drops might be due to the given item simply having a lower chance to drop than players think (unless the drop really is bugged; which has happened). But yeah who knows.
