In either event, you simply need to run enough L20+ quests (in Eberron) to collect 20 Tokens of the Twelve, take them to Lahar (alongside the Twelve building) and trade them for the appropriate Heart: Wood for TR, Blood for RR.

honestly its what you want to make out of the game. there are some people who only go to cap and stay there. some do the tr aka reincarnation train.

If this is a first-life toon, I'd recommend two TRs as soon as you can collect enough Tokens for the Hearts, for a couple of reasons. First, it gets you up to the maximum stat points (36 point builds; first Tr puts you at 34 whether you already had 32 point builds unlocked or not; second TR gives you 36 for that toon thereafter). Second, it allows that toon to open hard or elite quests at will thereafter.
After that, there's more room for preference: Run to 30 and pley at-cap content forever, run to 30 and ETR, run to 30 and then ETR followed immediately by TR, or continue to TR and gain past life feats with the goal of eventually attaining Completionist, Triple Completionist or Racial Completionist.
Before investing many lives into Epic TRs, I too would recommend purchasing Epic Destinies. They greatly increase the power of your epic toon directly and the effect improves (less dramatically) as you unlock additional destines, which is generally done through ETRs.
