Can you overlook graphics that haven't improved that much since then, but instead focus on your character and the game play ?
Are you planning to VIP a bit, or are you fine with playing the content you had before only? (content which probably hasn't changed vastly, outside of Champion mechanics)
Do you plan to solo, drag someone from 'back then' to play, or find a new group?
Lastly, 20+ is going to largely be dull without the first expansion, which VIP will not cover. Will you be willing to get the expansion before judging 20+ content, or want to make new character from scratch and delay the issue?

There have been a lot of Quality of Life changes from back then.. a lot of Spell-like Abilities to smooth out SP issues, two large revamps to enhancements to further customise (and power creep) characters, things like giving armour and shields the ability to mitigate some damage, and so forth. The addition of Saga Rewards to give a reason to run chains of quests and not just cherry pick the best xp/min ones. The barest basics of the game are pretty much the same, so it isn't a change there, but the theorycraft is different and your ideas of then might not carry forward.
i.e. a remake/new character is probably better than salvaging your old char. Or push to 20 and TR the character. (You can easily get a true heart to reincarnate by running the challenge instances, so that won't cost you)

In sum: if you had fun before, you could very well find fun again. It's simply a matter of 'has your tastes changed/aged? Somethings are as fun now as when you left them, some are bitter with nostalgia, and others are 'how the hell did I ever enjoy this?'.
At the least, just redl the game and putter around a bit. It'll only cost you bandwidth and a bit of time to do so.
