He doesn't mention anything about achievements. He's only concered with the "Ghost (Never Detected)" checkbox NOT being checked.
There were a handful of posts about achievements, by people who mistakingly thought the discussion was about the Ghost achievement, as you did. However, the vast majority of the thread has actually been about playing the Flooded District in high chaos while never being detected ... in just that mission ... and NOT about achievements.
Does that clear it up for you?
You do understand, don't you, that high chaos and sneaky are not mutually exclusive? Killing hostiles doesn't mean you can't do it quietly and unseen (undetected).
Let me try to explain it this way ... Whatever I'm doing (in game), I like to do it without being noticed, whether I'm killing someone, avoiding obstacles or stealing loot. Kinda like a ghost ... quiet and unseen. I did a few playthroughs attempting to remain concealed and undetected while killing all hostiles in the mission ... in every mission. So have many other players. It's not really so weird, at all.
That's obviously not what I meant and it's been like 6 months since I played this game last. I was remembering High Chaos as running around all Rambo-like not giving a crap, and not just for killing people. Yes, you can get High Chaos and Ghost, good for you. Here, you may have this.

I also now hate weepers more than rats.
In the following video, I had finished mission 6 in high chaos but played the Flooded District with no kills, to minimize the risk of being detected. At 7:20, as I leave Daud's base, the battle music plays, even though I obviously was never detected by that point, nor afterward. Still, the Ghost box was not checked.

TL;DR: Stop time as soon as you enter the Gateward Tunnel, then quickly possess a rat, move away slightly, and wait until possession ends. Make sure the combat music is not played and you can save your game at any time (the save game menu item is not stricken out).
I've been doing a lethal ghost run and stuck at the same point as the OP. Thanks to this thread and lots of googling, I've made the following observations:
1. When you are detected, you can't save your game for a while: the save game item is stricken out in the menu.
2. When you enter the Gateward Tunnel on high chaos, when the combat music plays, you can't save your game while it is playing.
3. You can't be detected when time is stopped. However, stopping time after detection doesn't cancel it, obviously.
4. If you stop time immediately after entering the Gateward Tunnel, you can save your game, and the combat music is not played. However, once the effect ends, the music is played and you can't save your game for a while. Meaning, it's not an instant detection, it's a nigh-instant detection.
5. You can't be detected while possessing something or someone.
So I stopped time and then possessed a rat from a swarm that guards the nearby bone charm. Then I moved away slightly, so I wouldn't be in the middle of the swarm once possession ends. I don't know how long exactly do you have to wait to get rid of that detection, I waited for the mission objective to complete or for the possession to end, I'm not sure which. The good news is that it worked!
I don't think the whole Granny Rags conundrum matters at all, but in case if anyone curious, I dealt with it in the following way:
1. Enter the cave, run to the left and up the stairs. No sneaking at this point, but I'm not sure whether Granny saw me or not.
2. Take her cameo, open the furnace.
3. Put it inside the furnace, turn it on and then run and jump out the window. I even think she actually saw me in the process, but she was just ranting, without any detection marks, neither gray nor red. I think she doesn't enter the enemy mode instantly.
4. Hide next to the window and wait until she teleports away slightly and moans and groans.
5. Lean and headshot her. Probably bend time and blade would work even better, given all those useless potions lying around.
As for the gear retrieval part, I entered the refinery and left it through the building with the dead overseers, but I was very careful around it: sniped one or two whalers on the roofs. Maybe they were the ones watching the building and the bridge, so they didn't detect me. Anyway, there was neither any combat music nor detection markers at that point.
The only enemies I did not kill were the tallboys because it was easier to sneak around. I even got rid of all the weepers in the tenement building. And still got the ghost checked. The same tactics without that bend time and possession runaround did not give me the ghost on the previous two attempts, so I think that's exactly the reason of this annoying and subtle bug. Happy ghosting!
That IS a nice find.
I did pretty much what you said and was pleased to find that it does work. A nice work-around.
I began mission 7 with high chaos and went through the Flooded Distrcit with no kills and no detections. Using your guidelines, I got both No Kills and Ghost checked.
